Physical frailty can predict the vulnerability for community-dwelling older adults as the functional performance declines. Recently, nurses and health care professionals have implemented the strategies to enhance physical capacity among frail older adults. Even the clinical guidelines of physical activity and exercise recommendations are widely published, older adults still engage in a low level of physical exercise. It is notable that exercise intervention is effective way to prevent frailty. This review indicates that a combination of aerobic, resistance exercise, and multicomponent exercise can improve physical performance such as increase muscle strength, balance, muscle mass, gait speed and prevent falls. Frailty is a reversible condition. Therefore, encouraging older adults to perform physical exercise is needed to maintain physical functioning as well as promote quality of life. Nurses play a significant role in frailty recovering both in hospitals and in aged care facilities. © 2020, Research and Development Institute Suan Dusit University. Method: Participants were Thai people aged 15 years or older. A total of 25,758 Thai people were selected through a stratified random sampling method. The study was a secondary analysis of a national cross-sectional study using the data from the Thailand Smoking and Drinking Behavior Survey 2014. Results: Thai people were regular drinkers (13.69%). Regarding gender, 26.54% of men and 2.72% of women wen were regular drinkers. The risk factors for regular drinking among Thai men included family members and close friends consuming alcohol, being aged 25-44 years, living in Northem Thailand, being a skilled or unskilled worker, smoking, and income of 10,001-30,000 Thai bahtimonth (31 Thai Baht (THBYU.S. dollars). Protective factors against alcohol consumption were living in Southern Thailand and being unemployed/a student. For women, the risk factors for regular drinking were family members and close friends drinking as well as smoking. Being married, living in a nonmunicipal area, holding a diploma or bachelor's degree, and being unemployed/a student were protective factors against alcohol drinking among women. Factors associated with alcohol consumption among Thai people vary by gender. Even though cardiac patients tried to seek the ways to promote their health, they might face with the difficulties to change the behavior. Numerous studies showed that self-efficacy plays a crucial role as a buffer to perform activity as reflect personal beliefs and confidence. HRV is a predictor of prognosis of heart failure and cardiac functions. However, little is known about the relationship of physical activity, HRV improvement and autonomic regulation in heart failure of older adults. The purpose of this state of science review is to synthesize the evidence that shows the effects of physical activity or exercise, HRV and physiological outcomes. The literature searched includes 12 studies from four electronic databases (PsycInfo, Pubmed, Scopus and CINAHL). The findings revealed that physical activity or exercise will improve HRV. This review showed that physical activity improves cardiac autonomic function (nerohormornal activation, sympathetovocal balance, baroreceptor reflex) and inhibits alpha 2 antagonist to improve HR reserve and RR intervals and prevent premature ventricular contraction (PVC). However, the level of physical activity and intensity are not clear. Tailored interventions or community-based programs will be effective in order to maintain physical activity and functional outcomes. A higher HRV is related to the functional capacity compared to the lower.
